That one only does splitting takes up a whole rack space and uses XLR connectors...

If you don't mind making up XLR to 1/4" cables...here's one that does mixing and splitting...

PS you can make up a splitter quite quickly if you purchase a

1- Radio Labs ST-DA3
1- Hammond aluminum box
5 - 1/4" jacks (1 input and 4 outputs)
1 - 2.2mm x 5.5mm Power Jack (isolated!)

Here's one that I made up for a pedal board that has a switch to mute the guitar and feed a tuner...

Drill a few holes in the box for the jacks and to wire it up, the DA-ST3 has screw terminals so the only soldering you have to do is to the 1/4" and power jacks. Close her up, connect it to your voodoo lab pedal power +12 VDC and then you can buy any mixer (passive or active) of your choice...maybe the...

Here's another one I just found, the Galaxy JIB/S. It's a passive 4-way splitter, and the price is certainly right, but I don't know anything Galaxy Audio or the quality of their products.

I was surprised to discover Whirlwind doesn't make a small 3 or 4 channel unit like this. I've been using a Selector for years, but that's an A/B/Y box and the OP would need a 3rd signal. Another option would be an Axess BS-2 splitter, which does have 3 outputs, but they're scarcer than hen's teeth and I'm not parting with mine. :)

Regarding mixers, though, I've been using an Alesis MultiMix 8 Line for a while and am very happy with it, but there are a lot of other companies that manufacture single-space line mixers that would do the job quite nicely. All depends on how much the OP wants to spend.

For the splitter side, I think that you will need an active (powered) spiltter for adequate isolation...

For the Mixer side, you can use a passive mixer.

You're looking at wiring up special cables again. Not that there's anything wrong with that if you're handy with a soldering iron, but if you don't want to make up the cables there are plenty of multi-channel single-space rack mount splitters available from manufacturers such as Rane, Samson, Behringer, etc.

You might also want to take a look at the Alesis Multi-Mix 8 Line mixer.
